Located on Indian River Memorial Hospital's fifth floor, the Transitional Care Program is a 28-bed unit for individuals who require skilled nursing or rehabilitation after having an acute-care hospital stay. The Transitional Care Program's goal is to return the patient to his/her lifestyle by improving functional ability. It is certified by the Agency for Health Care Administration.

The staff includes a team of doctors, nurses, therapists, speech pathologists, dietitians, social workers and pharmacists who assist and support each patient on an individual basis. Patients receive one-on-one therapy for speech, occupational, physical and recreational rehabilitation.

Rehabilitation is provided for patients recovering from total knee, total hip, or spine surgery, gastrointestinal surgery, ostomy surgery, vascular surgery, stroke, and patients with cardiac and pulmonary problems.

All rooms are private with a bath and telephone.
